Prescriptions and Health Forms

Please call the office for medication refills. We require twenty-four hours notice for   ADD/ADHD medication refills   Due to government regulations, ADD/ADHD prescriptions must be picked up.  They cannot be called in to pharmacies nor can they be mailed. Please call the office twenty- four hours before you need your prescriptions.

Due to the volume of health evaluation forms(school physicals, sports participation forms, college physical, school administering of medication forms and shot records) that are requested, it will take twenty- four hours for completion.  We can mail or fax these forms, if asked.  Forms brought in during a physical exam will be completed during the visit.
